Síndrome disejecutivo y sintomatología
esquizofrénica: implicaciones en rehabilitación
psicosocial

Introducción. El efecto más importante a evaluar en
rehabilitación psicosocial es la autonomía del paciente, que
en gran medida viene condicionada por su capacidad de
funcionamiento en actividades laborales y por su grado de
participación social. Este resultado depende de múltiples
factores que interaccionan entre sí. El objetivo del presente
estudio es conocer la asociación existente entre la
disfunción ejecutiva y el parkinsonismo medicamentoso con
la sintomatología psicótica en la esquizofrenia ambulatoria
susceptible de rehabilitación psicosocial.
Métodos. Se ha realizado un estudio transversal, tomando
una muestra de 30 pacientes ambulatorios con el
diagnóstico de esquizofrenia y un grupo control de 15 sujetos
sanos. Se aplicó una batería neuropsicológica de
instrumentos relacionados con la memoria operativa y con
la función ejecutiva: trail making test A y B, test de Stroop, test, color trails, Wisconsin card sorting test (WCST), index of
color trails, Wisconsin card sorting test (WCST), índice de
memoria de trabajo del WAIS-III y BADS (behavioural
assessment of the dysexecutive syndrome). Se evaluó la
sintomatología psicótica mediante la escala PANSS y el
extrapiramidalismo con la escala de Simpson–Angus. Se
realizó un estudio correlacional y mediante regresión lineal.
Resultados. Los esquizofrénicos rindieron peor que los
controles sanos en las pruebas de función ejecutiva. La
puntuación en la batería BADS y en el WCST, junto con el
extrapiramidalismo permiten explicar el 63% de la variancia
de la puntuación PANSS negativa y el 62% en el caso de la
dimensión desorganizada.
Conclusiones. El síndrome disejecutivo se relaciona con las
dimensiones sintomatológicas negativa y desorganizada de
la esquizofrenia. Podría ser un síndrome neurocognitivo
nuclear que ayude a comprender los déficit funcionales en la
enfermedad.

Introduction. The most important effect to evaluate in
psycho-social rehabilitation is the autonomy of the patient,
which to a great extent is determined by his functioning
capacity in vocational activities and by his degree of social
participation. This result depends of multiple factors that
interact to each other. The objective of the present study is
to research the association between executive dysfunction
and drug induced parkinsonism with psychotic symptoms in
schizophrenia outpatients susceptible to psycho-social
rehabilitation.
Methods. A cross-sectional study was carried out, taking a
sample of 30 schizophrenia outpatients and a control group
of 15 healthy subjects. A neuropsychological battery of
instruments related with working memory and executive
functioning was applied: trail making test A and B, Stroop
working memory of WAIS-III and BADS. Psychotic symptoms
were assessed by means of the Scale PANSS and the
extrapyramidalism with the scale of Simpson-Angus.
Correlational and linear regression study were made.
Results. Schizophrenics performed worse than the healthy
controls in the tests of executive function. The score in BADS
battery and WCST along with the extrapyramidalism
explain 63% of the variance of PANSS negative score and 62
% of that in the disorganized dimension.
Conclusions. The dysexecutive syndrome is related to the
symptom dimensions of schizophrenia negative and
disorganized. It could be a nuclear neurocognitive syndrome
that contributes to understand functional deficits in the
disease.
